I've emailed them about the lack of National Association anywhere in the 'consultation' (survey).  I've asked them to add it in ASAP.

I'm meeting them (the CAA person in charge of designing this new registration and education scheme and a couple of people from 'Sparck' (the company that seems to be building it) on Friday at the DfT in London).

Also meeting DfT earlier on Friday about registration and competency testing in general.  EASA's latest wording doesn't require model flyers (IE members of associations) to be registered so I will be pushing for us to be exempt.  CAA is apparently against gold plating EU regs and this would be exactly that so fingers crossed.

Also talk of us having to do a competency test every 3 years is outrageous.  You don't have to be retested for your driving licence or your PPL, etc.  So we're very much against that.

How can they start building it without having a specification? It must already exist somewhere if there's a company already lined up to do the development...  in which case, what has already been decided it should incorporate?

Yes I noticed that too about the lack of a mention of National Associations. The closest the form got was a mention of registering which is not the same as joining or being a member of. I used the "Other" options to mention model associations.
It struck me that the form was more like a first draft put out simply for testing the form, rather than using it for "consultation to help us build an online registration service." To me it shows a lack of understanding of unmanned flying and the differences between commercial and recreational use of UAVs.

I agree with you too about regular testing. Whilst I think the idea is not so bad in principle I cannot see the need for it if it is not deemed necessary for a driving licence. Whilst we gain driving experience over the years, how many of us pick up bad habits or fail to keep up to date with regulation changes? If the government deems it unnecessary for driving re-testing for 10s of millions of drivers, why should the minority activity of model flying be lumbered with this?

Could I suggest asking the CAA about the potential effects of Brexit on model flying? I know from information circulating within the aviation industry that the future is unclear but that the CAA is concerned enough to have created a microsite to address the concerns of aviation professionals.
